Loan and Distribution forms can be completed, signed, and sent to Benefit Equity, Inc. Be sure to put your company name associated with the Retirement Plan on the form. For example, if your company goes by another name (dba )give us the legal name and the dba, if you are a subsidiary give us the parent company name and your subsidiary.
